Antioxidant Level Testing: Measure Your Body’s Defenses

You’ve heard that antioxidants are important, and you probably take a multivitamin or eat your berries. But do you actually know whether your body’s antioxidant defenses are adequate? Most people are guessing. Antioxidant level testing removes the guesswork — and the findings consistently surprise even health-conscious patients.

What Antioxidant Testing Reveals

Antioxidants are molecules that neutralize free radicals — unstable compounds generated by normal metabolism, inflammation, environmental toxins, radiation, and stress. When free radical production overwhelms antioxidant capacity, the result is oxidative stress — a state that accelerates aging and is now recognized as a central mechanism in virtually every major chronic disease: cardiovascular disease, cancer, neurodegeneration, diabetes, autoimmune conditions, and more.

The key antioxidant systems in the body include enzymatic defenders (superoxide dismutase, catalase, glutathione peroxidase) and non-enzymatic ones (glutathione, vitamin C, vitamin E, coenzyme Q10, alpha-lipoic acid, carotenoids, and polyphenols). These systems work synergistically — a deficiency in one impairs the regeneration of others. Glutathione, in particular, is often called the ‘master antioxidant’ because it is central to recycling vitamin C and vitamin E and is the primary intracellular defense against oxidative damage. (Read more about oxidative stress and chronic disease on PubMed.)

Why You Can’t Assume Your Antioxidant Status Is Adequate

Dietary intake of antioxidants is necessary but not sufficient for adequate tissue levels. Genetic variants — particularly in the enzymes that synthesize and regenerate glutathione (GSTM1, GSTP1, SOD2, NQO1) — significantly affect individual antioxidant capacity independent of diet. Chronic illness, toxic exposure, intense exercise, and psychological stress all dramatically deplete antioxidants. And many commonly prescribed medications — statins, metformin, proton pump inhibitors, and antibiotics — are known to deplete specific antioxidants.

Without testing, targeted supplementation is blind. With testing, we can identify exactly where your defenses are weakest and address them precisely — which is both more effective and more cost-efficient than taking handfuls of supplements hoping something helps. (The Institute for Functional Medicine’s overview of oxidative stress provides helpful context on why individualized assessment matters.)

What Antioxidant Testing Looks Like in My Practice

A comprehensive antioxidant assessment in my practice typically includes:

  • Reduced glutathione and oxidized glutathione ratio — the most clinically meaningful measure of cellular antioxidant status
  • 8-hydroxy-2′-deoxyguanosine (8-OHdG) — a urinary marker of oxidative DNA damage, indicating current oxidative burden
  • Coenzyme Q10 (plasma ubiquinol/ubiquinone ratio) — essential for mitochondrial energy and cardiac protection
  • Plasma vitamin C, vitamin E (alpha and gamma tocopherol), and carotenoid levels
  • Superoxide dismutase (SOD) and catalase activity
  • Organic acid markers of oxidative stress: 8-isoprostane and lipid peroxides
  • Genetic analysis of antioxidant enzyme variants to identify constitutional vulnerabilities
