Delusions: Fixed False Beliefs and the Biology Behind Them

A delusion is defined as a fixed, false belief that is held with absolute conviction, is resistant to counter-evidence, and is out of keeping with the person’s cultural background. Delusions may be paranoid (believing one is being followed or persecuted), grandiose (believing one has special powers or status), somatic (believing one’s body is diseased or infested), or referential (believing that unrelated external events carry personal meaning). They represent one of the most distressing experiences in psychiatry both for the person experiencing them and for their loved ones.

Delusional disorder and delusional states within schizophrenia, mania, and other conditions are typically treated with antipsychotic medications. But too frequently, the biological cause driving the delusional state is never investigated, and the medication manages the symptom while the underlying driver continues unchecked.

The Neuroscience of Delusion Formation

Delusions are understood neuroscientifically as aberrant salience: the brain’s pattern-detection and threat-assessment systems (centred in the dopaminergic mesolimbic pathway) assigning excessive significance and meaning to neutral or coincidental stimuli. When dopaminergic mesolimbic tone is elevated either through excessive dopamine release or through impaired dopamine clearance, random neural firing gets “stamped” with excessive significance, and the reasoning processes of the prefrontal cortex fail to adequately challenge or correct the resulting false inferences.

This is a biological process. The prefrontal cortex’s ability to evaluate and correct delusional thinking is directly dependent on its chemical and energetic state, which is, in turn, influenced by nutrition, toxins, hormones, and immune function.

Biological Drivers of Delusional States

1. Excess Dopamine Activity, Methylation and Copper Dysregulation

In overmethylated individuals (identifiable through whole blood histamine and methylation markers), SAM-e activity is elevated, leading to excess dopamine synthesis and release. This is the neurochemical correlate of paranoid and grandiose delusions. Copper excess, which inhibits dopamine beta-hydroxylase and thus prevents dopamine clearance, similarly amplifies mesolimbic dopamine tone. Both are testable and represent among the most biochemically specific and actionable drivers of delusional states.

2. B3 (Niacin) Deficiency

The orthomolecular psychiatry tradition demonstrated that high-dose niacin intervention produced meaningful remission in schizophrenic and delusional patients, particularly those in early-stage illness. Niacin is a methyl acceptor, meaning it draws excess methyl groups away from dopamine synthesis pathways. In overmethylated patients with elevated dopamine, this mechanism directly addresses the neurochemistry of delusion. Functional niacin status and methylation markers guide this assessment.

3. Autoimmune Encephalitis

Delusional thinking is a prominent feature of anti-NMDA receptor encephalitis and related autoimmune encephalitic conditions. The NMDA receptor governs prefrontal cortex function and error-correction processes. When antibodies block these receptors, the brain loses its ability to reality-test and correct aberrant inferences. Delusional states arising acutely, particularly with accompanying motor abnormalities or behavioural changes, warrant immediate autoimmune neurological evaluation.

4. Thyroid and Endocrine Dysregulation

Hyperthyroid states, hypothyroid states (such as myxedema madness, a rare but dramatic consequence of severe hypothyroidism), and steroid hormone dysregulation (including cortisol excess from Cushing’s syndrome, or extreme estrogen fluctuation) have all been associated with delusional thinking. Comprehensive endocrine evaluation is part of our standard workup for new delusional presentations.

5. Heavy Metal Neurotoxicity

Mercury poisoning historically produced the phenomenon of “mad hatter’s disease,” paranoid, delusional, and erethistic behavior from chronic mercury exposure. Lead and cadmium accumulation disrupt the prefrontal regulation of limbic dopaminergic activity. In any patient with a significant environmental exposure history and delusional symptoms, heavy metal evaluation is warranted.

6. Stealth Infections and Neuroinflammation

Borrelia (Lyme disease), neurosyphilis, and certain viral encephalitides can produce delusional states as part of neuroinflammatory presentations. A complete infectious workup including specialized serology and relevant viral panels is important in any new, unexplained delusional disorder.

Understanding the Scope: What a Functional Evaluation Can and Cannot Do

Medical Disclaimer: It is important to be transparent: functional medicine evaluations and supportive protocols do not cure or reverse acute psychiatric psychosis, delusional disorders, or schizophrenia, nor do they replace primary psychiatric care, neurological evaluation, or prescription antipsychotic medications. Complex psychiatric conditions require comprehensive, coordinated management with your psychiatrist, neurologist, or mental health team.

Instead, the goal of a targeted functional investigation is to identify and address modifiable metabolic, nutritional, immunological, and toxic factors that contribute to neurological stress. By supporting cellular health and systemic balance, our objective is to help optimize overall well-being while you navigate conventional care.

Our Functional Medicine Approach

No delusional presentation that comes to our practice is evaluated without a systematic biological workup:

  • Methylation status and whole blood histamine.
  • Copper/zinc ratios.
  • Niacin functional markers.
  • Autoimmune neurological panels.
  • Full thyroid and hormonal assessments.
  • Heavy metal and environmental screens.
  • Infectious and inflammatory markers.

We work directly with the patient’s prescribing physician throughout the process to ensure safe, integrated care. Delusions represent the most extreme end of biological psychiatric dysregulation, and they deserve the most thorough biological investigation.
